Our Policies:
Appointments:
We kindly ask for at least 48 hours’ notice if you need to cancel or reschedule your appointment. Appointments canceled with less than 48 hours’ notice, or missed appointments, will be subject to a cancellation fee.
Surgery & Procedures:
A $750 non-refundable booking fee is required to secure your procedure date. Remaining balances are due 2 weeks prior to surgery.If surgery is rescheduled, fees may apply depending on how close the change is to your procedure date.
HSA / FSA Cards:
At this time, we do not accept HSA or FSA cards for services or products.
Cancellations:
Procedures must be canceled at least 2 weeks in advance. Booking fees are non-refundable.
Insurance Cases:
Changes made within 14 days may result in a $100 fee, which is the patient’s responsibility.
Additional Notes:
Product returns are accepted within 2 weeks for store credit
A $100 fee applies to returned checks
For safety and comfort, we ask that children not attend appointments
